Support Coordination

Support Coordination helps Participants to connect with the NDIS and access services and supports which aligns with their plan and goals. Support Coordinators monitor plan budgets and aim to build Participant’s capacity and capability in understanding their plan.

Support Coordinators aim to:

  • Understand the plan – identify how the funding is managed and ensure funding lasts the length of the plan.
  • Connect with supports and services – have knowledge of services in the community, collaborate with mainstream supports, exercise choice and control, connect providers and support services and meet the needs of the Participant.
  • Design support approaches – identify support preferences, design the correct mix of support to help the Participant pursue and achieve their goals through NDIS, community, funded and mainstream support.
  • Establish supports – create service agreements, service bookings, understand the pricing guide and identify barriers and risks to the plan’s implementation.
  • Coach, refine and reflect – build the Participant’s confidence and independence when navigating the NDIS.
  • Report to the NDIS – complete timely progress reports to ascertain the plan’s effectiveness.
  • Build capacity and resilience – allow the Participant to exercise choice and control and build their confidence and skills to implement their plan.
  • Crisis: planning, prevention, mitigation and action – support Participant with crisis management strategies around their NDIS plan.
